Once we contact you to get you scheduled please follow the following directions:
You will receive an email with a link to our patient portal via the email you provided.
Finish setting up your account on our Patient Portal and then login and complete the following:
1. Fill out all New Patient Paper work, by creating an account in our patient portal
2. Complete the Medical Symptom Surveys and Patient Intake Forms assigned to your account, these are located in your patient portal.
3. Upload copy of insurance card both front and back sides to the document section of the patient portal. Your insurance cared is required prior to setting up your appointment. Please complete your profile with the Insurance responsible party information. We can also take your FSA or HSA as payment for your services and supplements.
4. Once you have set up your account in our patient portal and have completed the intake questions and added your insurance information. Send us a message via the patient portal that you are ready to schedule your new patient visit. You will be contacted within 5 business days to schedule your appointment. When calling the office it is essential that you leave a voicemail because we will not know that you called. If you leave a voicemail you will get a call back.
You will visit with our provider and discuss your complete medical history and current symptoms, and your health care goals. During this visit we will decide what testing is needed to develop your personalized plan of care.
You will be scheduled for a follow up visit approximately 2-3 weeks after your initial visit. At this visit we will develop your plan of care based on your individual needs and goals. We typically see our patients every 3-4 months. Follow up visits can be completed in-office or via telemedicine.
We have a Labcorp Representative located in our office. We typically order labs at your new patient appointment and during follow up appointments. Labcorp bills all insurance carriers for there services. Your insurance company will only pay for testing that they deem medically necessary. Some functional medicine tests are not considered medically necessary and may not be covered under your insurance plan.
If you are prescribed hormone therapy, labs are required prior to all prescription refills, especially with testosterone as this is a controlled substance. You will always be prescribed enough medication to last you over 90 days to get you to your next appointment.
All Lab visits Require an Appointment. These appointments are double booked and patients will be seen in the order of appointment first and then the order of arrival.
It is important that you maintain your primary care provider. We do not have walk-in or same day appointments. You will need your Primary Care Provider for your chronic conditions and medications, pain medications, sick visits, and referrals. We work with your primary care provider to help optimize your health and help you meet your health care goals.
For patients who elect this form of hormone replacement therapy once health screening and required testing is completed we will schedule you for your hormone pellet placement. This appointment last 30 min. Please do not be late or we will have to reschedule your appointment and you will be responsible for a $50 No Show/Cancellation Fee.
During your visit you will be provided enough medication to last until your next appointment. Most hormone replacement therapy require lab testing prior to your next visit to be able to accurately adjust your medications to establish or maintain optimal therapeutic levels. If you need to reschedule please schedule for earlier than your next appointment due time or you will run out of medications, and we can not refill these medications without proper testing and appointment to review current symptoms.
If you need a refill of a medication prescribed by one of our providers, please go to your patient portal and send in a request for refill. We typically provide enough medication to get you through your next visit and you should not have to do this majority of the time.
Some medications especially controlled substances require you to be seen every 90 days prior to refill. These medications will not be refilled without an appointment and if necessary bloodwork. Your provider will discuss this with you during your visits.
If you are requesting a medication that our providers have not prescribed for your previously this will require an appointment with your provider prior to the prescription being sent to the pharmacy. This ensures appropriate use of medication based on your personal needs to help you reach your health goals.
